Customer Declaration, Consent & Disclaimer

I, [Customer Name], owner / authorized user of Vehicle Registration Number [Registration No.], voluntarily request an independent review of my vehicle repair estimate, invoice, or maintenance recommendation.

I understand and agree that:

1. The review is based solely on the information and documents that I provide.
2. Unless specifically mentioned in writing, no physical inspection, dismantling, diagnostic scan, or road test has been carried out by the Service Provider.
3. The Service Provider provides an independent professional opinion only and does not certify that any repair is unnecessary, defective, excessive, or fraudulent.
4. Repair recommendations made by the authorized workshop may be based on observations not available to the Service Provider.
5. The Service Provider is not responsible for any future mechanical failure, safety issue, breakdown, accident, warranty dispute, insurance claim issue, or reduction in vehicle performance arising from decisions made by me.
6. I understand that delaying or declining repairs may affect the safety, reliability, emissions compliance, or longevity of my vehicle.
7. I remain solely responsible for deciding whether to approve, reject, delay, or modify any repair recommendation.
8. The Service Provider does not guarantee any reduction in repair costs, dealer approval, reimbursement, warranty continuation, or dispute resolution outcome.
9. I authorize the Service Provider to discuss my repair estimate with the dealer or workshop if required.
10. I confirm that all documents submitted by me are authentic and complete.
11. I agree that any technical opinion or report provided is confidential and intended solely for my personal use in connection with this vehicle.
12. I release, indemnify, and hold harmless the Service Provider, its directors, employees, consultants, and representatives from any claims, losses, damages, liabilities, costs, or expenses arising from my reliance on the advisory opinion, except to the extent caused by fraud or wilful misconduct of the Service Provider.
13. I have read this document carefully, understood its contents, and voluntarily accept all its terms without any coercion.
